Paid parking on the street for € 1.50 per hour, max. € 6 per day.

PRIVACY DECLARATION

In order to be able to guide and treat you properly and carefully, I will create a file in which I will store information about you. Your file contains personal data (such as name, address, postal code, place of residence, e-mail address, telephone number), notes about personal circumstances and health, and notes about the conversations and treatments conducted.
I keep the information about you secret, unless I can not on the basis of a legal obligation.
I also do my utmost to guarantee your privacy. This means that I handle the personal and medical information provided to me with care. And I make sure that unauthorized persons do not have access to this data.
If third parties request information from your file, I will only provide it with your explicit permission. In the case of peer review, I use anonymized data.

COMPLAINTS PROCEDURE

I am taking every effort to treat you as a client in the best possible way. In the unlikely event that you are not satisfied with the way I treat you or how I have treated you, I invite you to discuss this with me. So we can look together what we could do about it.

If you do not want or cannot discuss the complaint with me, or if you want independent support in this regard, you can contact Klachtenportaal Zorg (portal for Healthcare Complaints). Klachtenportaal Zorg is an external organization that I am affiliated with to help resolve dissatisfaction and complaints about my healthcare.

Submit your complaint using the complaints form on the website of Klachtenportaal Zorg. You can also send an e-mail to This email address is being protected from spambots. You need JavaScript enabled to view it.. After you have sent your complaint or dissatisfaction, the Klachtenportaal Zorg will assess whether it meets the conditions for handling it and which procedure is appropriate. If information is missing or unclear, they will contact you to make it complete and to ensure that the complaints procedure is properly started.
